Mga Kinakailangan (Requirements): 1. Application Form na sinagutan at nilagdaan (duly accomplished and signed) 2. Diagnosis ng bata at rekomendadong therapy program mula sa isang doktor o propesyonal na kinikilala sa Pilipinas (child’s diagnosis and prescribed therapy/ intervention program from a recognized Developmental Pediatrician, Neurologist, Psychologist or SPED Diagnostician in the Philippines) 3. Sertipikasyon ng pagiging maralita mula sa Barangay kung saan nakatira ang pamilya (Certificate of Indigency from the Barangay Hall where the family resides). 4. Valid ID ng magulang tulad ng ID na ipinagkaloob ng gobyerno o ng kompanya (parent's valid ID such as a government-issued ID or company ID) 5. PWD ID ng aplikante (PWD ID of applicant) |
Mga Alituntunin (Guidelines): -Mga kumpletong aplikasyon lamang ang maipoproseso. (Only complete applications will be processed.) -Ipagbibigay-alam ng Perfect Soul Center for Education and Healing (PSCEH) sa aplikante at pamilya nito ang resulta ng kanilang aplikasyon. (Applicants and their families shall be informed by Perfect Soul Center for Education and Healing (PSCEH) regarding the decision on their application.) -Mangyaring iwasan ang pagtawag sa telepono ng PSCEH upang mag-follow up ng aplikasyon.
